In the 20th century, when she traveled time to influence Usagi's choices as Sailor Moon at the Galaxy Cauldron, she went by "Chibi Chibi."
When still in her form as Cosmos, she often uses "Umi" as an alias, as it is a possible female name reading for the kanji 宇宙 (cosmos, the universe) and also a common enough name as to not be questioned or noticed.

Age and Date of Birth: Appears to be in her early twenties, although her true age is unknowable. She celebrates her birthday on June 30.
 
Used Canons: Manga, as Sailor Cosmos exists almost exclusively in the manga.
 
Appearance: Cosmos is of average height, but her excellent posture and carriage makes her seem taller and gives her a presence that can be imposing. She has a lean build, elegant and shapely. Her skin is a rosy peach colour. She has masses of white hair. When not styled, it pools around her on the floor. She most commonly wears it pulled up in two high pigtails, each topped with a heart-shaped bun. Her bangs are parted down the center of her head, where they curl tightly upon each other in mirrored "S" curve shapes to frame the eight-point golden starburst typically seen on her forehead. Strands of the thick, voluminous hair fall before her ears and curl at the nape of her neck. Her eyes are a deep, dark blue with silver undertones. In some lighting situations they can seem almost black, with just a glimmer of silver. Despite her white hair and narrow white eyebrows, her eyelashes are thick and dark. She has pink lips almost permanently set in a calm, gentle expression. Cosmos is one of those people that tends to make people feel at ease in her presence, rather than being intimidated by her beauty; she has a comforting and relaxing aura.

Cosmos is almost always seen in her uniform as Sailor Cosmos. She only opts for her non-transformed appearance when absolutely necessary. In that case, she selects to wear whatever is most common and will not stand out or look unusual in the place and time she is in.

Another form that Cosmos once assumed is that of Chibi Chibi: a pink-haired toddler with rosy peach skin and chubby cheeks. This child has bright sapphire-blue eyes and magenta hair. Her bangs are styled similarly to Cosmos', and she share the same heart-shaped hair buns. However, Chibi Chibi's hair is much shorter, emerging from the buns not as full pigtails but as short curls that barely reach her chin. She has similar curls before her ears and at the nape of her neck. Chibi Chibi often wore a school uniform or sailor-styled casual clothing.

Cosmos is able to disguise herself in many ways. A regular appearance of hers is that of a small white kitten with an eight-point gold starburst on her forehead. The kitten is the same shade as Cosmos' hair and has the same eye colour as her human form.

Personality: Cosmos borders on omniscience; she is not quite all-knowing and all-seeing, but she is certainly much-knowing and much-seeing. To cope with this, she has developed unrivaled patience and serenity. She aims to make others feel safe and calm around her, particularly if she has a message to deliver that might be difficult to understand or unpleasant. Her long life has taught her when to share information and when to reserve it for herself. She aims to make others feel comfortable at all times, regardless of whether or not she can or will help them.

For the most part, Cosmos operates on a philosophy that avoids interference. When possible, she does not identify herself, her history, or her mission. If she must do so, she provides only the bare minimum of details. She strives to avoid imposing her values and ideals on other people and civilizations. She does not discuss technology or concepts with those who have not yet developed such things independently. Cosmos believes that every planet and every people must make their own choices and perform their own actions towards future success. She does not think that good comes from simply doing something for another world or telling another world how to act.

Cosmos has not always followed this plan, but she has seen so much disaster and heartache arise from the best of intentions. Adoption of this philosophy helped her learn from the mistakes of her own past. This can be hard on her sometimes, as Cosmos is a very mothering and mentoring person. She likes to care for and protect. If she could share her wisdom with others so that they could learn from her experiences, rather than having to discover these personal truths themselves, she would do so.

To many she seems almost too agreeable, suggesting a weak will. Cosmos will not engage in arguments or push forward her own opinion unwillingly. She tries to support the choices made by those around her, even if they are choices she would not make. However, if there is something that she truly believes in and feels must happen, she will manipulate the situation as best she can to isolate the elements that she knows could upset things most--as she did when she prevented other sailor soldiers from engaging in Sailor Moon's confrontation with Chaos.

Sometimes others perceive her as hardhearted. This is typically because Cosmos avoids attachment to people and worlds, as she knows that she will both outlive their lifespans and be forced to leave. She also does this to guard herself against the soul-crushing sorrow and regret that she feels when she she fails to prevent some horrible event from occurring. She also knows that many entities that she meets and perhaps even grows to love will someday die. She has felt that emptiness before, and every time it makes her wonder if she can truly go on.

Her outward poise and calm disguises her most crippling weakness: self doubt. Cosmos does not believe in herself. She remembers her failures more poignantly than her successes. She constantly questions her own choices. Should she have interfered and saved that child's life? Should she not have protected that planet? There is no one else to provide feedback or guidance for Cosmos; she has never met another being like herself.

History: Sailor Cosmos is the final sailor soldier, one with the universe through the combined Lambda Power of untold sailor crystals. She has seen many wars and many times of peace. She has watched many deaths and witnessed many births. She wanders through time and space, seeking to bring order where there may be Chaos. Chaos and Cosmos are two sides of the same coin, always fighting for dominance.

In the 20th century, Sailor Cosmos traveled back to the time before Sailor Moon's battle with Chaos, hoping to convince the powerful sailor soldier to destroy the Galaxy Cauldron and prevent Chaos from being eternally reborn. She disguised herself as a toddler called "Chibi Chibi" to gain Usagi's trust and provide comfort to her loneliness. It was at that final battle, watching Sailor Moon chose to embrace rather than fight Sailor Galaxia and to allow the Galaxy Cauldron to create new stars even as it created new darkness, that Sailor Cosmos remembered the power of hope for the future.

Since that moment, although it is now distant and barely remembered, Sailor Cosmos has sought not to eliminate the darkness, but to nurture hope for the future.

Other Noteworthy Facts: Sailor Cosmos sometimes makes allusions to being descended from or having been Tsukino Usagi, but her actual past is murky. It's possible that in one timeline she could be an ascended form of Sailor Moon. There is no way to prove or disprove this. It's clear that she at least honors the memory of that wonderful young girl who realigned her path.

Senshi Name: Sailor Cosmos
Realms of Influence: The universe, light, rebirth, order, and purity
Henshin/Transformation Phrase: She does not need a phrase or item to transform into Sailor Cosmos, having achieved Lambda Power via the combined energy of all sailor crystals throughout the universe.

Senshi Fuku: Sailor Cosmos wears a white dress with a white sailor collar and a short pleated skirt. The white sailor collar is edged with three silver stripes. Attached below to the collar, to her shoulders, is a floor-length white cape of lightweight, translucent and somewhat sparkly fabric with excellent drape. Her sleeves consist of three padded bands of gold fabric. The bands farthest from the dress's arm scythes are adorned with a row of pearls with white feathers arranged in small wing shapes. In the center of her chest is a large round medallion marked with a eight-pointed gold starburst. The lower hemisphere of the circle is encircled with pearls, ascending in size from the bottom of the circle to the halfway point. From the pearls sprout large white feathered wings. Like all the wings on her uniform, they appear semi-translucent at times. The skirt portion of her dress is pleated, with a large central box pleat. On this central box pleat is an array of multicolored ribbons, arranged radiating from the top center of the pleat. The ribbons' colours are, from left to right: black, violet, pink, red, yellow, green, teal, navy, and pale blue. The point where the ribbons converge is covered by another winged medallion, identical in design to the one on her chest, only slightly smaller. Two narrow white ribbons drape down to Sailor Cosmos' ankles, attached somewhere behind the lower central part of that winged adornment.

Her shoes are white pointed-toe high heels with pearls and wings decorating the shoe at the back of the foot, above the heel. Around her neck is a simple white ribbon, accented with an eight-pointed gold starburst at the center front of her throat. She sometimes wears silver rings with large white pearls on all of her fingers except her thumbs. She wears three earrings in each ear--white pearls, with a larger pearl higher up from the lobe and smaller pearls in descending order. The smallest pearls, worn in her earlobes, have dangling delicate silver chains suspending three tiny white pearls. A brilliant eight-pointed gold starburst sparkles in the center of her forehead, decorated on either side with a line of delicate white pearls. Above her bangs, nestled towards her hairline, she wears two mirrored pearl and wing decorations. Sailor Cosmos' heart-shaped hair buns are ornamented with round white shields. An uneven gold border around the shields, thicker near the top of the shields and inlaid with a pattern of circles, seems vaguely Art Nouveau in style.

Powers: Sailor Cosmos is extremely powerful, but her abilities are not always useful in battle. None of her abilities require that a phrase be spoken.

• Nullifying Barrier - Sailor Cosmos is able to protect herself and others within a 3-meter radius from attacks, regardless of the element or power level used. She can activate it almost instantly. It does not require a weapon or other item.
  
• Sailor Crystal Awakening - Provides a spark that awakens a dormant sailor crystal and enables the holder of that crystal to transform. She typically provides the transformation item, if needed. She is easily able to see holders of sailor crystals; to her, they appear to have a level of brightness glowing from them that correlates to their sailor crystal power.
  
• Lambda Power - Pure energy from the Cosmos Crystal. Can be used offensively or defensively, and Sailor Cosmos is able to control the level of energy output. She does not know if there is a limit on the power she can channel and whether there might be a level that could overload and kill her. Cosmos is unsure of whether she can die.
  
• Power Sharing - Sailor Cosmos is able to transfer proportionate amounts of Lambda Power from herself to other sailor soldiers. This can be used to permanently provide a power up for another sailor soldier. She is also able to receive and refocus additional energy from any other sailor soldier, acting like a intensifier.
  

Weapons and Magical Items:

• Cosmos Staff - Sailor Cosmos is often seen with a thin white staff, taller than the sailor soldier herself, topped with a three-dimensional version of the medallions worn on the front of her uniform. At the top of the staff is a white round bead, topped by a larger gold round bead, and above that a massive opalescent orb that sometimes appears translucent. The bottom hemisphere is decorated with rows of pearls that are smallest at the lowest point of the orb and larger near the center line. From those pearls emerge two feathered wings. Atop that orb is a smaller transparent sphere containing an eight-point gold starburst. The bottom end of the staff has a large gold bead atop a smaller white finial bead.
  

 
RP Sample: Space. Vast and silent. When she closed her eyes, she could feel it flowing past her, like laying down in a woodland stream and letting the cool water wash over you. Always moving; always expanding. This was her home and her being.

The sensations of stars dying out and of stars being born echoed within her.

She knew that Cosmos Guardian was near even before she opened her eyes. Sailor Cosmos smiled down at the sprite, small enough to cup in her own hand. Her lone companion.

"Thinking of her?" The guardian of the Cosmos Crystal's voice always reminded her of bells.

"Mm-hm," she murmured, relaxed and safe within this ocean of potential. Of course she was thinking of her: Sailor Moon. That brilliant star. How long had it been since that moment facing Chaos? She couldn't recall. She just remembered the essence of it: the shining heart filled with love and hope. She was changed by it.

"Do you worry... about Chaos?"

Sailor Cosmos stared ahead, the swirl of stars reflecting in her deep blue eyes.

"As long as there are new stars... I don't fear the dark," she said softly. Other words, told to her so many lifetimes ago, floated through her memories. That message of hope. She would keep it shining brightly in her chest, until someday they met again.

I'm a bit hesitant to add that, because on the same vein of "Is Cosmos some form of Usagi?" I think whether Sailor Cosmos was always a soldier or became a soldier in a specific event is not established and barely hinted at. I'm willing to add that information as storyline notes if I were playing Cosmos in a particular storyline where that part of her history became essential.

I see where you're coming from. Feel free to keep that part ambiguous and leave her history section as it is.

[HC Event] The Lunar Chronicles Phase VI - Lunar Eclipse: It is now thousands of years into the future since the Crystal Tokyo era. Senshi have come and gone and the battle between the side of Cosmos and Chaos have finally reached its peak. A new senshi, Sailor Cosmos, whose power rivals that of the legendary senshi Sailor Moon, has emerged as the ultimate champion of the light side. Just like Sailor Moon had been, she is supported by nine senshi in her team in her war against Chaos.

However, this battle is harder than all of Sailor Moon's old battles combined. And the stakes are much higher. The world's balance is at risk, depending on which side ultimately wins. Is there even a true winner after all, or will everything be lost and wiped clean for the world to restart? As long as the Galaxy Cauldron exists, even if one side prevails for now, the other will soon be reborn and the whole thing starts all over again...

Sailor Cosmos is tired. She has experienced many losses, some from her own senshi team. Some among the people she dearly loves. She just wants it to end. Will she take a risky move to bring an end to the suffering?

Chaos has manifested itself and took the ultimate form of Sailor Chaos, a senshi of destruction with incredible power. It's going to be a fight of the strongest soldiers of light and darkness in this ultimate showdown.

This event will explore what happens leading up to Cosmos' decision to travel back in time to Usagi's era and try to destroy The Galaxy Cauldron.

Character Notes: In this timeline, Sailor Cosmos has not yet traveled back in time. She has been fighting for what feels like forever, traveling across the universe in an attempt to protect all life from the damaging influence of Chaos. Cosmos no longer remembers her origin, only recalls small facts about herself: She may have once come from Earth, or at least... someone she cared deeply for did. There was a choice, she thinks, but even if she concentrates she doesn't remember what it was. Catching glimpses of a particular shade of blue eyes fills her with melancholy. She believes in the power of love, and thinks that everyone can be saved if only she tries hard enough.

She is almost solely focused on her mission, a thing which feels like it has no end: push back the forces of Chaos.

As Sailor Cosmos has not yet tasted devastating defeat, she is not as aloof or fatalistic as her canon manga self. However, she is clearly growing jaded and exhausted as the battle rages on. It is only the support of her teammates, other soldiers that are her friends and her family, that buoys her spirits. She may have once known and loved others, but that does not diminish how she feels about those who ended her solitude and joined her on her quest. She couldn't bear to ask any of them to follow her, but they chose to do so. Sailor Cosmos loves them all like sisters—and like children. They're precious to her, and she feels thankful that they have elected to be by her side. Although she can accept being alone, when it is necessary, it chills her core and makes everything in her life feel more bleak.

The recent battles amplified this feeling. So much death. So many poor, young sailor soldiers trying so hard to protect their worlds, their lives ended so suddenly. So many veterans re-entering the conflict, only to have another mission become their last. So many civilians on so many planets, terrified and at the mercy of forces they barely comprehend.

As the war continues, Cosmos tries to put on a brave face for the soldiers fighting with her, but she can feel her own confidence eroding. It seems endless, without any hope of closure. How could she have chosen to do this? She may not have the typical concerns of a human—eating, sleeping, and other bodily needs virtually optional now that she has become what she is—but there is a kind of tiredness creeping into her mind, tainting her perspective. She can't imagine what true rest would even feel like. Does she even deserve it? Are her actions doing anything—helping in any way? Is she just prolonging something that will come to pass no matter what she does?
